Many organisations believe that success depends on making the right decision. While that is true, an equally important factor is when the decision is made. A well-informed decision that comes too late often has little value. In operational environments, timing can be the difference between preventing a problem and simply documenting its consequences.

An organisation suffering from Operational Blindness experiences the opposite. It spends much of the available time searching for information, confirming facts, assigning responsibility, and obtaining approvals. By the time action begins, the opportunity to prevent the incident may already have disappeared.
